Human Resources Notice on Employee's Maternity Leave

Dear [name of employee],

Congratulations and thank you for telling me about your pregnancy and the date that your baby is due. I am writing to you about your maternity leave and pay.

As we have discussed, you are eligible for [number of weeks] maternity leave. Company policy on maternity leave is as follows:

[Insert Maternity Leave Policy]

Given your chosen start date of [insert date], we anticipate your return to work on [date]. Your maternity pay will be the amount of [insert amount], this is a per day amount based on your monthly salary. You are also entitled to maternity benefits under the company's health plan.

If you want to change the starting date of your leave you must, if at all possible, inform me at least a month before your proposed new start date.

If you decide to return to work before [insert date leave ends], you must give me at a few days notice.

During your maternity leave, please make sure to keep in touch. [Name of employee] is filling in for some of your work tasks and he/she might need assistance or instruction from time to time.

If you decide not to return to work you must still give me proper notice. Your decision will not affect your entitlement to your severance pay and other employee benefits.

If you have any questions about any aspect of your maternity leave, please do not hesitate to get in touch with me. Congratulations to you and your family as you anticipate the arrival of your little one.
