Improving Tardiness and Attendance Record Letter

Dear Mr. Gaston,

Your tardiness record has been a glaring concern on our daily attendance reports. We have reviewed all your submitted excuse letters. We would like to express our concern about your health, whether you are still physically fit to take on your duties. It would be best to see your personal physician for what constantly ails you. Or you may agree to see the company doctor within the week for a thorough medical check-up. Due to your skillset, you have proven more often than not how valuable you are to your team. The request to improve your attendance record comes from the senior department supervisor.
